Candidate Forms You Need To File To Run For Office

Candidates must file two forms prior to officially qualifying: Form DS-DE 9 and DS-DE 84.

  1. Form DS-DE 9, Appointment of Campaign Treasurer and Designation of Campaign Depository: the first document required to be filed with the filing officer to become a candidate. Form DS-DE 9 must be filed with the filing officer prior to opening the campaign account.
  • Designates the office for the candidate
  • Appoints a campaign treasurer
  • Designates a campaign depository

Nothing prohibits a person from announcing his or her intention to become a candidate prior to filing a Form DS-DE 9, however candidates may not accept any contributions, make any expenditures, or circulate petitions until the DS-DE 9 is filed with the Qualifying Officer.

Special District candidates who do not intend to collect or expend campaign funds are not required to file the DS-DE 9. An Affidavit of Intent must be submitted if a Special District Candidate chooses this option.

  1. Form DS-DE 84, Statement of Candidate: must be filed by the candidate within 10 days after filing Form DS-DE 9. This form states that the candidate has received, read and understands the requirements of Chapter 106, F.S.

Additional Candidate Filing Forms:
Form DS-DE 83, Statement of Candidate for Judicial Office: must be filed by the judicial candidate within 10 days after filing Form DS-DE 9. This form states that the judicial candidate has received, read and understands the requirements of The Florida Code of Judicial Conduct.
